Direct Support Professional:
Provide support, training and assistance to individuals in their Hammer homes. Duties include building relationships with our clients, helping them to realize their dreams (large and small), activity planning, transportation, cooking, cleaning and home maintenance.
Assistant Program Manager:
Provide same duties as a Direct Support Professional plus assist the Program Manager with administrative duties.
Program Manager:
Supervise daily operations of group home, which include staff supervision and scheduling, facilitating team meetings, crisis intervention and direct care.
Our homes are located in the western suburbs of: Crystal, Eden Prairie, Edina, Golden Valley, New Hope, Maple Grove, Minnetonka, Plymouth, St. Louis Park, and Wayzata.
