Services
Hammer programs meet individual
physical, psychological, social, religious, educational,
vocational and recreational needs. Our administration,
staff and volunteers provide those we serve with lives
full of dignity and respect.
When we design a program, we
think of the unique gifts, desires, and needs of the people
we serve. We enjoy helping people discover who they are
and what brings them joy. In the past, we emphasized helping
people with disabilities to acquire new skills. We now
realize how important it is to help people to discover
who they are.
The
supports we provide to those served includes:

relationships with family and friends

and supporting daily living skills

assistance in free-time activities

residents through life transition

in meeting health needs
We also offer several
other programs, including:
- In-Home
Support – Licensed Service Program
Our experienced team of professional staff is available
to provide a full program of licensed services in an individual’s
private home.
We
use a Person Centered Team approach to determine each
individual’s desires and needs then develop an
individualized health, safety, and desired outcome plan.
We facilitate regular progress reports with a commitment
to supporting each individual in determining their own
path and achieve their goals.
Services
can range from helping to increase an individual’s
independence in self-care, relationships skills, self-determination,
community access, and much more.
Staff
members maintain their employment status at Hammer,
receiving compensation, benefits, all training required
for licensed providers, and supervision through our
organization.

In-Home
Support - Personal Support Services
We also offer Personal Support In-Home Services for
individuals who receive services through a traditional
waiver. We assist individuals by providing quality care,
helping them to determine and achieve their desired
outcomes. Whether their goals are to increase independence,
be more involved in the community, or create natural
supports within their lives, our staff members are dedicated
to assisting individuals in achieving their dreams.
You may choose to hire staff from our experienced team
of professionals or we may be able to employ the staff
that are currently providing services for your family
member. Hammer provides staff with benefits, compensation,
and training.

Flexible
Case Management
Hammer provides Flexible Case Management services for
those who utilize Consumer Directed Community Services
(CDCS). Our Flexible Case Managers can assist you in
navigating the ever-changing waiver system. We can help
you to develop an Individualized Community Support Plan
to best meet the needs of your family member. We can
assist you with the implementation of the Community
Support Plan, and provide a wide range of supports such
as, providing resources, finding and hiring direct support
staff, and developing creative strategies to meet individual
needs and desires.

Supported
Employment Services
Hammer provides supported employment services to assist
an individual in finding and maintaining employment
that matches their needs and desires. We can assist
in assessing employment options and help to prepare
an individual for all aspects of potential employment.
We also offer on-the-job training, monitoring, and development
of outcomes to increase an individual’s marketability
and value to their current or potential employer.
