Hammer offers Semi-Independent Living Skills Services uniquely designed to meet the needs and desires of people with disabilities in their family’s home.

Our experienced staff members are committed to helping individuals to increase their independence and self-determination. Hammer staff members work with the individual and their support team to develop a custom care plan including personalized annual outcomes and a health and safety plan. This plan may be accomplished through traditional methods such as an established person-centered planning tool or an alternative planning tool better suited to the individual. We regularly report the progress of outcomes and are committed to assisting each individual to determine their path and achieve their desired goals. Our staff members can also provide additional assessment as necessary.

Our staff members have completed all training required for licensed providers and continually update their education through our state-of-the-art training program. When a client has an additional need, Hammer also offers specialized training specific to those needs. Our SILS staff members maintain their employment with Hammer. They are supervised, paid, and receive benefits through Hammer.

Hammer’s Semi-Independent Living Skills Services can be funded through the DD waiver program, using county funds, or through private pay.

supported employment services

Hammer provides supported employment services which match the individual’s needs and desires in finding and maintaining employment. We help to assess employment opportunities and prepare the individual for all aspects of potential employment. We assist with on-the-job training and monitoring and also assist in developing outcomes to increase an individual’s value to their current or potential employer.

These services are funded through waiver programs such as DD, CADI, and TBI and are also available for private pay.

personal support services

Our experienced team of professional staff members can assist an individual by providing quality care and helping them to determine and achieve their desired outcomes. Whether an individual desires to increase his/her independence, become more involved in the community or simply needs more daily support, Hammer staff members are dedicated to assisting individuals to achieve their goals and realize their dreams.

While Personal Support Services are not licensed and outcomes are informal, staff members receive extensive training and are employed, supervised and receive compensation and benefits through Hammer.

Personal Support Services are available to those individuals who are receiving services through the DD waiver or through private pay.
