Contracted Case Management Services are an exciting new way for Hammer to support people with disabilities. In February 2007, Hennepin County chose Hammer as one of a few private agencies authorized to provide case management services as a new alternative for families

Hammer’s experienced well-trained case managers strive to work with individuals and their families to develop strong, positive relationships and creative strategies to assist them in developing effective personal life plans

An effective case manager is a vital link to help families navigate the maze of the social service system while promoting consumer choice and self-determination. While our case managers work for Hammer, they also represent and are required to follow county and state standards

Hammer’s case managers have at minimum a Bachelor’s degree from an accredited university and meet the state standards for the provision of case management services. Each case manager receives ongoing training to stay abreast of changes and new opportunities related to case management. Currently, Hammer serves case management clients who are eligible for the Community Alternatives for Disabled Individuals (CADI) Waiver and the Developmental Disability Waiver (DD, formerly MR/RC). These clients live within Hennepin County and the surrounding areas
