Job Title: Standard DSP

Supervisor: Program Manager

Job Summary:

  • The Direct Support Professional (DSP) position directly supports individuals with intellectual and developmental disabilities. The people we serve live in Hammer’s group homes or apartment programs with the support of care teams who provide on-going assistance, teach independent living skills, support health and personal cares, help with household tasks, and offer social and emotional support.

Essential Functions:

  • Communication: Reads email and communication book every scheduled shift. Demonstrates effective communication with co-workers and families.
  • Training: Completes and passes required training on-time.
  • Schedule: Is on-time for every scheduled shift, meeting attendance policy standards.
  • Daily Documentation: Regular logging in Direct Service Record (DSR), incident reporting and communication book.
  • Essential Core Functions: Personal cares, household tasks, medication administration, providing on-going social and emotional support for individuals including community participation.
  • Adherence to Hammer Residences Policies, including, but, not limited to: limited cell-phone usage on-site while providing care to Hammers individuals.

Job Specifications:

  • Must be 18 years of age or older
  • Valid Driver’s License, current vehicle insurance and a good driving record
  • Pass a background check through the Department of Human Services
  • Technical skills. Must be comfortable and proficient in using computers and/or smart technology.
  • Flexible schedule availability
  • Good written and oral communication skills
  • Life coaching, mentoring skills
  • Perform within a team framework
  • Working knowledge implements positive engagement techniques
  • Work within a defined set of policies, procedures or guidelines
