Our experienced Support Planners (formerly known as Flexible Case Managers) will provide person-centered, high quality services to meet the needs of families and support them in navigating the waiver system. Hammer’s Support Planning Management Services are available to families who use Consumer Directed Community Services (CDCS).
Our Support Planners strive to work with individuals and their families to develop creative strategies to meet ongoing needs and desires. We can assist in developing a wide range of supports to provide continuity of care, to access resources and to coordinate staff selection and hiring. We develop person-centered plans and a curriculum for staff and families including sign translation services if needed. We are also available to help with all of the required paperwork for CDCS such as Community Support Plans and Health and Safety/ Risk Management Plans.
Hammer’s Support Planners have a combined experience of over 20 years of working with children and adults with disabilities. They are college-educated, well-trained and certified by the State of Minnesota. In fact, the annual training requirements for Hammer’s Support Planners exceed the guidelines set by the State of Minnesota. Our Support Planners are experienced in working with all the waiver programs (DD, CADI, CAC, TBI, EW and AC) available under Consumer Directed Options.
